### Definition

Alias for the failure-mode half of [By Design Or By Default]. Every system, workflow, standard, and role expectation in the operation is either designed with intention or running "by default" -- on whatever was pre-selected by the operator, a prior operator, a software vendor, an industry norm, or nobody in particular. The default is not neutral; it is a decision already made by someone, producing behavior right now, whether the current operator knows it or not.
